As CDC safety guidelines change to accommodate current conditions, our plans will continue to adjust as needed. • I have not had a fever, cough, shortness of breath, difficulty breathing, chills, muscle pain, sore throat, or new loss of taste or smell that cannot be attributed to another health condition
• I have not had contact with a person known to be infected, potentially infected, or exposed to someone infected with COVID-19 within the previous 14 days.
• I understand that it is recommended that I wear a mask indoors when not eating, drinking or presenting as a featured speaker during event
